Dr. Most's Surgical Approach

Deep plane facelift surgery, as performed by surgeons like MD Sam Most, involves precise dissection beneath the SMAS layer to achieve comprehensive facial rejuvenation. The technique requires releasing key facial ligaments — including the zygomatic and masseteric ligaments — which allows for natural repositioning of descended facial tissues without excessive tension on the skin.

Unlike traditional facelift approaches that primarily tighten skin and superficial tissues, the deep plane method addresses the root cause of facial aging by repositioning the entire facial soft tissue complex as a unified layer. This sub-SMAS dissection plane provides several advantages: it preserves facial nerve branches that run superficially, maintains blood supply to the skin flap, and enables more dramatic yet natural-looking improvements in the midface and jawline. The procedure typically begins with careful marking of anatomical landmarks, followed by infiltration of tumescent solution to facilitate dissection and minimize bleeding. Surgeons experienced in this technique understand that the deep plane extends from the lateral face into the central face, requiring confident navigation around critical structures including facial nerve branches. The key to successful deep plane surgery lies in complete release of retaining ligaments, which allows the SMAS-platysma complex to redrape naturally without tension. Recovery from deep plane facelift generally involves less bruising than traditional techniques because the dissection occurs in a relatively avascular plane beneath the SMAS. Most patients experience peak swelling during the first week, with significant improvement by the two-week mark when many feel comfortable returning to social activities. The deeper repositioning of tissues also contributes to longer-lasting results, with many patients maintaining their improved facial contours for 10-15 years or more. San Francisco surgeons practicing this technique often emphasize the importance of customizing the approach based on individual anatomy and aging patterns.

One reviewer specifically praised the meticulous approach to emergency laceration repair, noting how the surgeon 'took his time carefully' during the procedure — a quality that likely translates well to elective aesthetic surgeries where precision is paramount. The review sample also demonstrates the practice's responsiveness to patient concerns, with one reviewer updating their feedback to acknowledge how 'the team reached out directly and made sure the situation was fully resolved,' suggesting a commitment to patient satisfaction beyond the initial procedure. Multiple reviews reference the 'kind, personable' nature of the surgical team, with patients expressing complete trust in both the surgeon and staff — crucial factors for those undergoing facial rejuvenation procedures. Within San Francisco's competitive aesthetic surgery market, MD Sam Most operates as one of five surgeons offering deep plane facelift procedures, sharing the city's average Google rating of 4.8 stars. This rating parity suggests a generally high standard of care among San Francisco's deep plane specialists, with patient satisfaction levels remaining consistently strong across the cohort. The presence of five specialized surgeons in a single city indicates robust demand for advanced facial rejuvenation procedures among San Francisco's affluent population. All five surgeons maintain active Instagram profiles, reflecting modern practice management and patient engagement strategies that have become standard in aesthetic medicine. While specific experience levels aren't available for comparison, Dr. Most's 143 Google reviews suggest an established practice with substantial patient volume over time. The fact that none of San Francisco's deep plane surgeons explicitly advertise international patient services (according to available data) may indicate a focus on local and regional clientele, though this doesn't necessarily exclude international patients from seeking treatment. Among his peers, Dr. Most's review count ranks in the upper percentile, suggesting either longer establishment in the community or particularly active patient feedback culture within his practice. Deep plane facelift costs in San Francisco typically reflect the city's position as one of the most expensive metropolitan areas in the United States, with surgical fees generally ranging from the upper-middle to high end of national pricing scales. While specific pricing for MD Sam Most's practice isn't publicly available, San Francisco's aesthetic surgery market commands premium rates due to several factors: high overhead costs including surgical facility fees, the concentration of experienced surgeons, and the affluent patient demographic. The comprehensive nature of deep plane facelift surgery — requiring specialized expertise, longer operative times, and extensive anatomical knowledge — justifies higher fees compared to traditional facelift techniques. Patients should expect the total investment to include surgeon's fees, anesthesia costs (typically performed by board-certified anesthesiologists given the procedure's complexity), surgical facility charges, and post-operative care including follow-up appointments. Many practices structure their fees to include certain post-operative supplies and medications, though patients should clarify exactly what's included during consultation. When evaluating cost-to-value considerations, the longevity of deep plane facelift results (often lasting 10-15 years) can make the higher initial investment more economical over time compared to less invasive procedures requiring frequent repetition. Insurance typically doesn't cover elective facial rejuvenation surgery, so patients often explore financing options offered through medical credit companies. As of 2026, purchasing power parity comparisons show San Francisco's surgical costs remain higher than many international destinations, though patients often prioritize proximity and ease of follow-up care when choosing local surgeons.

Recovery Expectations

Recovery from deep plane facelift surgery with MD Sam Most or any experienced surgeon follows a predictable timeline, with most patients experiencing their most significant healing during the first two weeks post-procedure. Initial recovery (days 1-3) typically involves facial swelling and tightness, with patients wearing a supportive garment and managing discomfort through prescribed medications while keeping the head elevated during sleep.

Initial recovery (days 1-3) typically involves facial swelling and tightness, with patients wearing a supportive garment and managing discomfort through prescribed medications while keeping the head elevated during sleep. By the end of the first week, drainage tubes (if used) are usually removed, and patients return for their first post-operative appointment where initial sutures may be addressed. Week two marks a turning point where bruising begins to fade significantly and swelling starts to subside, though some patients experience varying degrees of facial tightness as tissues settle into their new positions. Most individuals feel comfortable returning to desk work and light activities by day 10-14, though strenuous exercise and heavy lifting should be avoided for at least 3-4 weeks to prevent complications. The deeper tissue repositioning characteristic of deep plane technique often results in less visible bruising compared to traditional facelifts, as the dissection occurs beneath the SMAS layer where fewer blood vessels are disrupted. Between months one and three, subtle swelling continues to resolve, and patients notice progressive improvement in facial contours as tissues fully integrate into their lifted positions. Final results typically manifest by month six, though minor refinements in tissue settling can continue for up to one year post-surgery. Patients should maintain realistic expectations about the recovery timeline and plan accordingly for social and professional obligations.

Pre-operative planning typically involves an initial virtual consultation, allowing international patients to discuss their goals and receive preliminary assessment before traveling. Essential medical records, including recent blood work and cardiac clearance if required, should be translated into English and forwarded to the practice before arrival. Post-operative care coordination is crucial for international patients, who should arrange for a companion during the first 48-72 hours and ensure their accommodation includes appropriate amenities for recovery such as extra pillows for head elevation and proximity to the surgical facility for follow-up appointments. Before departure, patients receive detailed wound care instructions and arrangements for suture removal if needed after returning home, with many surgeons providing virtual follow-up consultations for long-distance patients. Travel insurance that covers potential complications is strongly recommended, as is planning for the possibility of extended stay if healing requires additional monitoring.

Had a large laceration required emergency repair at ED. Dr Hazboun was more than amazing. He took his time carefully sutured the laceration together. I could barely see any visible stitches when it was done. Dr Hazboun is friendly, understands fully of patient's needs, and skillful. Just had my post-op checkup, suture removal was almost discomfort-free. Highly recommended if you are looking for a plastic surgeon that takes time understands your needs with excellent hands. The office staff was also great and efficient. Five stars for sure!🌟😃🌟👍🏻🌟🎉🌟🥳🌟

Update: After posting my review, the team reached out to me directly and made sure the situation was fully resolved. I really appreciated their willingness to address my concerns, and follow through to make things right. Initial Review: I recently had an under-eye vein treatment done here, and unfortunately, my experience was very disappointing. After spending $350 on the procedure, I expected to see a noticeable improvement. Instead, the vein under my eye looks essentially the same. When I brought up my concerns, I was told that the area looked “slightly” better, but that felt more like an excuse than an honest assessment. From my perspective, I’m not seeing the level of improvement that would justify the cost of the treatment. I had hoped for transparency and a results-driven approach, but I left feeling let down and dissatisfied with the outcome. Overall, I would not recommend this treatment based on my experience.
