Dr. Steiger's Surgical Approach
MD Jacob Steiger approaches deep plane facelift surgery by working beneath the SMAS layer, the fibromuscular sheet that covers facial muscles and provides structural support to overlying tissues. This surgical plane allows for comprehensive repositioning of descended facial tissues without creating tension on the skin, which distinguishes the technique from traditional facelift methods that primarily tighten superficial layers.
The sub-SMAS dissection involves carefully releasing ligamentous attachments that tether facial tissues, particularly the zygomatic and masseteric ligaments that contribute to jowl formation and midface descent with aging.
By operating in this deeper anatomical plane, the surgeon can reposition the entire facial soft tissue unit as one cohesive layer, maintaining the natural connections between skin, fat, and muscle that preserve facial expression and movement. Tissue repositioning during deep plane facelift addresses multiple aging changes simultaneously — elevating descended malar fat pads, reducing nasolabial fold depth, restoring jawline definition, and eliminating jowls through strategic vector adjustments. The technique requires precise knowledge of facial nerve anatomy, as the dissection plane passes near important motor branches that control facial expression, demanding meticulous surgical technique and experience.

Patients typically experience less bruising than with traditional facelift techniques because the deep plane dissection preserves more superficial blood vessels, though swelling in the midface and neck areas remains noticeable during this initial period. Week two sees significant improvement in comfort levels, with most patients feeling ready for light activities and social interactions while wearing concealing makeup if desired, though residual swelling continues to resolve gradually.
The deep plane technique's comprehensive tissue repositioning means that while early results are visible, the final aesthetic outcome continues to refine over several months as deeper tissues settle into their new positions. By weeks 3-4, most patients return to normal daily activities including light exercise, though high-impact activities and movements that significantly elevate blood pressure should be avoided until cleared by the surgeon. The three to six-month period represents the phase where subtle improvements continue as scar maturation occurs and any residual swelling completely resolves, revealing the full extent of facial rejuvenation achieved.

Years of Experience
20+ years
I want to share my experience with Dr. Steiger’s office. Before surgery, I felt very well taken care of and trusted that I was in good hands. I spent a long time researching surgeons and ultimately chose Dr. Steiger because of his reputation and credentials. Unfortunately, my experience after surgery has been very different. I waited many years and saved a significant amount of money for a septorhinoplasty to address both functional and cosmetic concerns. I did bump my nose during the healing period, and whether that contributed to my current concerns now that the healing period is over is something I cannot say for certain. What I can speak to is how my postoperative concerns were handled, which has left me deeply disappointed. I entrusted Dr. Steiger with this procedure and believed he would be there for me if any concerns arose, as any good surgeon should. Unfortunately, that was not my experience. My nose is now dented and more crooked than it was before surgery, and my breathing is significantly worse. When I brought these concerns to Dr. Steiger, I felt they were largely dismissed and attributed to the fact that my nose had some asymmetry before surgery. I was never expecting a perfect nose — I was expecting improvement, not something that looks more crooked and functions worse. As I have since learned, “as the septum goes, so goes the nose,” and without properly correcting and stabilizing the septum, it is very difficult to improve symmetry. I was also very concerned by the way my functional issues were addressed. I was told that my breathing problems were related to my nose being “smaller,” and that if I wanted, he could make my nose bigger again. I was not told that my septum was severely deviated despite having paid to have this corrected. I have since had this deviation confirmed by another physician. During my appointment, Dr. Steiger looked inside my nose and told me everything looked “fine,” even though the deviation is clearly visible inside my nostril. The only solution offered to me was filler to camouflage the appearance of the nose. I agreed to the filler at the time because it was presented as the only option to improve the situation, and I was desperate for a solution after spending so much time and money on this procedure. I was not educated about the risks of placing filler in the nose or that it would not address the structural issue with my nose. The filler also does not correct the functional problems I am experiencing, and it is not a realistic long-term solution to require ongoing filler treatments for the rest of my life when I paid for a rhinoplasty. I did try to address these concerns directly with the office before writing this review, but I ultimately left feeling dismissed and unsupported. Patients should not only consider a surgeon’s results, but also how postoperative concerns will be handled if something goes wrong. A surgeon’s true level of care is often revealed not when things go perfectly, but in how they respond when a patient needs help afterward.
Dr Steiger is a wonderful plastic surgeon. I could not be happier with my result. My face lift has made me look at least 10 years younger, and so natural. He is so nice to deal with, and totally professional. His staff the same. Overall a lovely experience!
Dr. Steiger and his staff are exceptional. He is extremely professional, and the entire team is friendly, attentive, and well organized. I felt comfortable and well cared for throughout the entire process. The results are excellent, and I highly recommend Dr. Steiger and his wonderful staff.
