- ✓Prescribed medications
- ✓Ice packs on cheeks only (not neck)
- •Keep head elevated 30–45° at all times
- •Apply cold compresses to cheeks every 20 min
- •Take all medications on schedule
- •Wear compression garment as instructed
- •Have a caregiver present for 24 h
- •Do not bend forward or lift anything
- •Do not remove dressings or drains
- •Do not eat solid food (surgeon instructions)
- •Do not take aspirin or ibuprofen
- •Do not drive
- ⚠Severe uncontrolled pain beyond prescribed medication
- ⚠Rapidly expanding firmness under one side of face or neck
- ⚠Difficulty breathing or swallowing
- ⚠Temperature above 38.5 °C / 101.3 °F
Surgery day swelling hasn't peaked yet. The dramatic change comes days 2–4 — not today. Focus on following instructions exactly.
- ✓Supervised bathroom visits
- ✓Soft liquids
- •Stay hydrated with water and broths
- •Keep all follow-up appointments
- •Photograph your progress daily
- •Rest completely — your body is healing
- •Do not look in the mirror expecting results yet
- •Do not sleep flat — stay elevated
- •Do not apply heat
- •Do not smoke (it slows healing)
- ⚠Bulging or rapidly expanding bruise on one side
- ⚠Numbness spreading across entire face
- ⚠Excessive drain output (>60 mL/hour)
What you see today is not your result. Swelling is still increasing and will peak over the next 48–72 hours.
- ✓Short house walks
- ✓Clear liquids freely
- •Continue cold compresses (20 min on, 40 min off)
- •Take arnica supplements if surgeon approved
- •Walk gently around the house to prevent blood clots
- •Sleep with 2–3 pillows
- •Do not touch or massage your face
- •Do not apply makeup or skincare
- •Do not panic at the bruising — it's expected
- •Do not strain during bowel movements (take stool softeners)
- ⚠One cheek dramatically more swollen than the other
- ⚠Fever above 38.5 °C / 101.3 °F
- ⚠Pus or foul odor from incision sites
Bruising spreads via gravity — it may move toward your neck and chest over the next 48 hours. This is normal.
- ✓Short house walks
- ✓Soft solid food if surgeon approves
- •Understand this is the worst day — it only improves from here
- •Document with a photo for your own reference
- •Continue all prescribed medications
- •Focus on hydration and rest
- •Do not panic — swelling starts to fall from here
- •Do not bend at the waist
- •Do not expose face to any heat source
- ⚠Hematoma — rapidly expanding firm swelling under one side only (call immediately)
- ⚠Fever with chills
- ⚠Skin colour changes (pallor, blue tint) in skin near incisions
Day 3 is statistically the peak swelling day for deep plane facelift. You will look measurably better by day 7.
- ✓Stairs (with caution)
- ✓First drain often removed today
- •Attend drain removal appointment
- •Begin gentle lymphatic drainage massage if surgeon approved
- •Continue elevation while sitting and sleeping
- •Shower if cleared by surgeon (no water on face)
- •Do not let drain removal appointment slip — retained drain risks infection
- •Do not exercise beyond gentle walking
- •Do not eat salty foods (sodium increases swelling)
- ⚠Drain site shows redness expanding more than 2 cm
- ⚠New pain spike after it was improving
Bruising spreading to your neck and chest is gravity pulling pooled blood downward — not a complication.
- ✓Suture removal (days 5–7)
- ✓First outdoor outing (with hat/sunscreen)
- ✓Washing hair with assistance
- •Attend suture removal appointment
- •Apply broad-spectrum SPF 50+ sunscreen to scars
- •Begin gentle facial expression exercises if surgeon approves
- •Take before photos to compare to your day 3 photos
- •Do not pick at healing suture sites
- •Do not expose scars to direct sun without SPF protection
- •Do not drive (if still on opioids)
- ⚠Suture sites that won't close or show pus
- ⚠Fever at this stage (late infection)
Yellow-green bruising is a positive sign — haemoglobin breaking down into biliverdin (green) then bilirubin (yellow). Resolution is underway.
- ✓Concealer over healed incisions
- ✓Work from home (desk job)
- ✓Cooking light meals
- •Apply silicone scar sheets once incisions are fully closed
- •Use tinted moisturiser or concealer to feel more confident
- •Resume normal diet gradually
- •Do not drive yet if taking prescription pain medication
- •Do not attend high-stress social events
- •Do not use retinol or exfoliants near incisions
- ⚠Incision areas reopening or weeping at 10 days
You are 'concealer-ready' at day 10. Light makeup over closed incisions is now possible for most patients.
- ✓Return to desk job / office
- ✓Light social activities (restaurant, quiet gathering)
- ✓Makeup over all healed areas
- ✓Light walking outdoors
- ✓Driving (if off all opioids)
- •Celebrate the 2-week milestone
- •Resume gentle facial movements to prevent stiffness
- •Book your 1-month follow-up if not already scheduled
- •Continue daily SPF 50+ on scars
- •No strenuous exercise
- •No bending to lift heavy objects
- •No direct sun exposure to incisions without SPF 50+
- •No alcohol (swells remaining tissue)
- •No aggressive scar massage yet
- ⚠Infection signs: red streaks extending from incision, fever, pus at 2 weeks
Week 2 is the 'restaurant-ready' milestone. Most patients return to desk jobs and can socialise with light makeup.
- ✓Return to office
- ✓Light cooking, housework
- ✓Reduced-salt diet can be relaxed
- ✓Video calls and Zoom meetings
- •Begin gentle scar massage (circular motions) once healed — ask surgeon
- •Photograph progress at week 3 — compare to day 7
- •Continue silicone scar sheets nightly
- •Do not return to gym yet
- •Do not lift objects over 5 kg / 10 lb
- •Do not skip scar treatment — weeks 3–6 are peak collagen remodelling
- ⚠New lumps or hardness that's growing rather than shrinking (fat necrosis or seroma — call surgeon)
Week 3 is often when 'the fog lifts' — energy returns, and the result becomes recognisable.
- ✓Low-impact exercise (walking, stationary bike at 50%)
- ✓Most social activities
- ✓Light travel
- •Resume gentle facial exercises
- •Enjoy the emerging result — this is exciting
- •Monthly progress photos from this point on
- •No contact sports
- •No heavy lifting (>10 kg / 20 lb)
- •No swimming (incision exposure to pool chemicals)
- ⚠Significant facial asymmetry at 4 weeks — one side dramatically different (call surgeon)
At 4 weeks, the result is exciting but some swelling remains. The contour will continue to refine over months 2–3.
- ✓Full exercise (jogging, weight training, yoga)
- ✓Swimming (if incisions fully healed)
- ✓Most contact sports (surgeon approval required)
- ✓Driving cleared regardless of medication status
- •Celebrate 6-week milestone — you've reached full activity clearance
- •Book 3-month follow-up appointment
- •Continue daily SPF — scars remain UV-sensitive for 12–18 months
- •No aggressive scar massage beyond gentle circular motions
- •No high-impact direct hits to face (boxing, martial arts)
- ⚠Unusual firmness or lumpiness that's increasing at 6 weeks
Week 6 is the 'full clearance' milestone. Most patients are now unrestricted in activity.
- ✓All activities cleared
- ✓Resuming full athletic training
- ✓Swimming and water sports
- •Continue scar treatment diligently — peak scar maturation is months 3–6
- •Maintain SPF daily habit permanently on scars
- •Don't abandon scar treatment — it takes 12–18 months for full maturation
- ⚠Virtually none at this stage — any aesthetic concerns are best discussed at follow-up appointment
In this phase, residual swelling resolves very slowly. Improvement now is about deep tissue remodelling, not surface swelling.
- ✓All activities
- ✓All skin care products (cleared with surgeon)
- •Book your 3-month follow-up if not yet done
- •Take progress photos to compare to pre-surgery photos
- •No specific restrictions
- ⚠Virtually none
You're approaching the 3-month milestone. Most of your final result is visible now, with subtle settling still to come.
- ✓All activities
- ✓Official 'before/after' photography
- ✓Return to full skincare routine
- •Book your 3-month before/after photography session
- •Continue SPF daily — this is now a permanent habit for longevity
- •Enjoy the result — most of the final outcome is visible today
- •No restrictions — maintain sun protection on scars
- ⚠None — if you have aesthetic concerns, discuss at your 3-month follow-up
Month 3 is when most surgeons take official 'after' photos. Subtle further improvement continues over months 6–12 as deep tissue remodels.

DEEPPLANE™ editorial standard
The DEEPPLANE™ Review Council is the name under which DEEPPLANE™ publishes its editorial standard; it is not a standing board of named surgeons. Our guides are written and edited in-house to that standard. Review is recorded per article, not claimed for the site as a whole — a guide names a surgeon as its reviewer only when that surgeon has signed it, and says so plainly when none has.
