Deep Plane Facelift vs Thread Lift: Which Is Right for You?

Deep plane facelift is a surgical procedure that repositions deeper facial tissues for 10-15 year results, while thread lift is a minimally invasive procedure using dissolvable sutures that lasts 1-3 years.

Thread lift costs $2,000-$6,000 per session compared to $35,000-$300,000 in the US (typically $35,000-$75,000) for deep plane facelift. However, thread lifts need repeating every 1-3 years, potentially costing more long-term.

Some patients use thread lift as a 'test run' to see how they feel about a lifted appearance. However, thread lift results are subtle and temporary (1-3 years), so they do not accurately preview deep plane facelift results. Previous thread lifts generally do not interfere with a future deep plane procedure.

PDO threads can be combined with fillers and Botulinum toxin, but timing matters. Most practitioners recommend waiting 2-4 weeks between procedures. Thread lift should never be performed on someone with active infection or immediately after other facial procedures. Combining a thread lift with a deep plane facelift is not standard practice.

Many board-certified facial plastic surgeons are cautious about thread lifts because the results are subtle, unpredictable, and short-lived (1-3 years) relative to patient expectations set by marketing. Reported complications include thread migration, palpable or visible threads under thin skin, dimpling, and — in rare cases — infection or thread extrusion requiring removal. Surgeons who perform surgical facelifts routinely see patients who tried threads first for moderate-to-severe sagging and were disappointed, since threads cannot release the retaining ligaments or address volume loss the way a surgical lift can. This does not mean threads are unsafe for the right candidate — mild early sagging in a patient wanting a temporary, no-downtime option — but they are not a substitute for surgery once sagging is moderate or worse.

For patients with moderate-to-significant sagging, a deep plane facelift is the standard long-term alternative: it releases the retaining ligaments and repositions the SMAS layer and deeper tissues rather than just cinching skin with sutures, producing natural, dramatic results that last 10-15 years versus a thread lift's 1-3 years. For patients with genuinely mild, early sagging who are not ready for surgery, a SMAS facelift or non-surgical volume restoration (fillers, biostimulators) may be a more effective bridge option than repeat thread lifts.

Thread lifts can provide a subtle, temporary improvement for very early, mild jowling by mechanically suspending the skin with barbed sutures, but they do not meaningfully correct moderate-to-pronounced jowls — the jowl fat pad and platysma muscle laxity that cause visible jowling require surgical repositioning (deep plane or SMAS facelift) to correct. Patients with established jowls who try a thread lift typically see minimal change and often move on to a surgical consultation within 1-2 years.
